まず、ご安心ください。正当にご利用いただいている限り、Content ID の申し立てによって利用許諾が取り消されることはありません。下記フォームからご連絡いただければ、確認のうえ解除に向けて対応します。First, don't worry. As long as your use is legitimate, a Content ID claim does not revoke your permission. Send the details below and we'll help request its release.

補足（任意）Notes (optional)

「メールで送信する」を押すと、入力内容を本文に差し込んだメール作成画面が開きます。そのまま送信してください。Pressing "Send by email" opens your mail app with the details pre-filled. Just send it as is.
